Transition Care Network specializes in mental health services for older adults and their caregivers. We understand the unique challenges aging individuals face — from medical complications to isolation and grief — and we meet those challenges with empathy, professionalism, and licensed expertise.
Individual Therapy for Older Adults
Our licensed clinicians provide one-on-one mental health counseling for seniors in the comfort of their own home or via telehealth. We address:

Dementia & Cognitive Behavioral Support
Working closely with families and care teams, our clinicians:

Teletherapy & In-Home Support
Whether in the comfort of home or on a secure video call, our therapists meet clients where they are, literally and emotionally. We offer HIPAA-compliant virtual sessions that are simple to access, even for those who aren't tech-savvy. For those who prefer or require in-person therapy, our clinicians travel to homes, senior centers, or assisted living communities across the DC–MD–VA region. We work around mobility challenges, schedules, and energy levels, therapy shouldn’t feel like a chore, it should feel like care.
